Medical Care

At Open Door, we provide excellent medical care and treatment.  The care that our providers deliver is coordinated and centered on the patient’s specific needs. In addition to primary care, we provide chronic disease management services for illnesses such as diabetes, hypertension, asthma and HIV.  Our providers are all board certified or eligible in their specialties.

Open Door received the highest level of recognition as a patient-centered medical home by the National Committee for Quality Assurance (NCQA) in 2009. A patient-centered medical home is where patients are at the center of their care team and communication is based on trust, respect and shared decision-making. In a medical home electronic medical records systems make it possible to track patient test results, review medicines and follow up with providers involved in the care of the patient. Patients and their families are empowered to participate in the decision making process with their medical team.
