  • Open Door has been designated as a Patient-Centered Medical Home and more than 80% of our patients had a visit with their primary care provider last year.
  • 92% of our two-year-olds are up to date with their immunizations. This is well above the national average.
  • Our WIC (Women, Infants and Children) program had a 95.4% breastfeeding initiation rate in August 2011.  We have had the highest rate in New York State for fourteen consecutive months.
  • Our WIC Program has the second highest redemption rate of Farmers’ Market checks in New York State at 76%, well above the national average of 54%.
  • Open Door is a leading member of the breast and Cervical Cancer Screening program that provides free breast, cervical and colon cancer screening to eligible people.
  • Open Door is participating in a Hypertension research Project with the Primary Care Development Corporation (PCDC), New York University and Columbia University funded by the Agency for Healthcare Research and Quality (AHRQ).  This project uses advanced features of Open Door’s Electronic Medical record to improve the care of Hypertension patients.
  • 62% of Open Door patients who have Hypertension are well controlled.
  • 75% of Open Door patients with Diabetes are controlled with  A1c levels less than 9, reducing their chance of secondary complications.
  • Open Door offers depression screening and self- management support at all annual physical examinations.
  • Asthma patients receive an Asthma Action Plan, which teaches them exactly what to do when they have an asthma attack.
  • Rapid HIV testing is available so patients can find out their results in as few as 20 minutes.  This testing is also available to our dental patients. Testing is encouraged annually.
  • Over a third of the HIV positive patients in central Westchester receive care at our facilities.
  • Open Door has a unique patient advocate program that provides support through group and individual counseling sessions for patients dealing with chronic conditions or the complex issues of health care.
  • In the last five years we have seen an overall increase of 23% in our total patient population.
  • We serve 2,760 children in our four school-based health centers alone, providing medical and dental screening services, and health education on nutrition, fitness and healthy lifestyles.
  • 60% of our patients are female.
  • 40% of our patients are children under the age of 19.
