External Accreditation and Recognition

  • Open Door is accredited by the Joint Commission.  Accreditation means that we have demonstrated a commitment to the highest level of patient safety and patient care; and met the nationally accepted benchmark of quality.
     
  • Open Door has been recognized as a Level 3 Patient-Centered Medical Home by the National Committee for Quality Assurance (NCQA).
     
  • Open Door has been included in the NCQA’s Diabetes Recognition Program to highlight excellent outcomes for patients with diabetes.  
     
  • Open Door received the prestigious Nicholas Davies Award for Community Health from HIMSS, the international healthcare information management systems society.

High Quality Care of Women and Children

  • 92% of our two-year-olds are up to date with their immunizations. This is well above the national average.  
     
  • Our School Based Health Centers provided care for 15,431 visits for children in five Port Chester schools during the 2011/2012 school year.  The children are provided with medical and dental screening services, and health education on nutrition, fitness and healthy lifestyles. 
     
  • Our WIC (Women, Infants and Children) breastfeeding initiation rate from April 2011 to March, 2012 shows our local agency total at average 95.9%. Region total is at 78.6 % and State total is at 74.8%. Our agency has the # 1 breastfeeding initiation rate in NYS.
     
  • Our WIC Program has the second highest redemption rate of Farmers’ Market checks in New York State for 2012 at 73.6%, well above the state average of 56.8%.  The highest redemption rate was 73.8%.
     
  • Open Door is a partner with the New York State Department of Health Cancer Services Program of the Hudson Valley which provides free breast, cervical and colon cancer screening to eligible women.  
     
  • All eligible pregnant women are able to enroll in Medicaid at Open Door. 
     
  • We deliver over 500 babies annually and provide care coordination services which include education and wellness activities throughout our mothers’ pregnancies. 

High Quality Care of Adults

  • 70% of Open Door patients who have Hypertension are well controlled.
     
  • 80% of Open Door patients with Diabetes are controlled with A1c levels less than 9, reducing their chance of secondary complications.
     
  • Rapid HIV testing is available so patients can find out their results in as few as 20 minutes. This testing is also available in our Dental Department. Testing is encouraged annually.  In 2011 we processed 7,640 screenings.

Care Coordination and Patient Navigation

  • Open Door has a unique patient advocate program that provides support for patients dealing with chronic conditions or the complex issues of health care.
     
  • In 2012, Open Door began the Transitions in Care Initiative. It is a communication system with our primary admitting hospitals and with our Medicaid managed care partner, Hudson Health Plan, to alert us when a high risk Open Door patient is admitted to the hospital. Our Transitions team contacts the patient and sets up a post discharge visit with their primary care provider. The focus of the visit is to make sure the patient has everything they need at home to ensure a full recovery, including an understanding of their medications, their treatment plan and the ability to manage this treatment plan. By establishing this open communication with these patients and ensuring they have access to their primary care doctor, the team is able to identify problems before they get out of control.

Wellness and Prevention Programs

  • Open Door offers wellness programs to promote healthy behaviors including exercise classes and healthy eating programs. 
     
  • Our Recess Rocks® children’s class encourages children to engage in physical activity in a fun, non-competitive environment.
     
  • Ways to Enhance Children’s Activity and Nutrition is a 6 week series of  workshops which covers healthy eating for families.
     
  • Nearly 40% of patients who signed up for the wellness program did not engage in regular exercise before starting.
     
  • Most program participants cite better overall health as being number one reason for wanting to engage in an exercise program.
     
  • Open Door partners with the Exercise is Medicine® and We Can!® national public health initiatives to further endorse and deliver our preventive medicine program.
     
  • The We Can!® Wellness Program is designed to help all employees reach their personal wellness goals through fitness programs and classes as well as opportunities to participate in free screenings and workshops on exercise and nutrition topics.
